1. What is the “Live Better! Wellness Club”?
As a member, you’ll get discounts on prescription drug alternatives, vitamins and other popular over-the-counter items and even a range of grocery products that promote more healthful eating for you and your family. You can also receive recipes that promote more healthful eating, and if you so choose, health and nutrition information tailored to your own individual needs. In addition, you can utilize the club’s secured e-mail network to address pharmacy and related questions to our clinical pharmacists, who will guide you in making healthy decisions.

We’re well aware of today’s “information overload,” and are committed to providing clear and useful communication that is easy to understand and apply to your purchasing and other lifestyle decisions. Likewise, the discounts you enjoy through your membership will apply to a popular variety of pharmacy and other products that are as appealing to you as they are beneficial to your health.


2. You don’t have prescription insurance coverage?  No problem.
The Live Better! Wellness Club can save you hundreds of dollars a year - by offering over 300 generic Rx products at $9.99 for up to a 90-day supply


3. Need to monitor the food you eat?  No problem.
The Live Better! Wellness Club will give you information and valuable discounts every day on foods that promote healthier eating.


4. Not sure what to make for dinner?  No problem.
The Live Better! Wellness Club will send you enticing yet healthful recipes and save you money on the ingredients.

Healthful living is about making healthy choices. Membership in the Live Better! Wellness Club will make it easier and more economical to make the best choices for you, your children, others whose health is your concern, even your pets! We invite you to join today, and begin receiving the money savings and valuable information we want to provide.


5. How do I join the Live Better! Wellness Club?
You join by filling out an application at any of our pharmacy counters, and paying a $4.99 enrollment fee. If you already have a bonus savings club card for our stores, a sticker will be afixed to that card designating your upgrade. You will also immediately receive a sign up bonus coupon good for $4.99 off your next $25 grocery order (redeemed at the regular checkout)…and in two days, your card will be activated to generate the new program’s regular discounts. If you do not have a bonus club card for our stores, you must first apply for one at the Customer Service Counter, and after receiving it, bring it to the pharmacy counter and follow the same process just outlined. The only difference in this instance is that as a new bonus savings club member, you will have to wait five days before beginning to receive the new program’s discounts.

Beyond the “Basic” upgrade just described, you may also choose to join the LIve Better Generic Prescription Plan, which makes available over 300 generic prescription products for Club Members at a cost of $9.99 for up to a 90-day supply. This is available to customers who do not have any other prescription drug insurance plan, and available through the same application process just explained. If you choose to join this additional program, you will be issued a separate Rx plan card – one for each member of your household who will use the generic drug program.

Note that while there is no additional enrollment fee to do this, you will be required to provide some basic information regarding your personal health background. This information will provide a necessary proÞle for our clinical pharmacists to consult in prescribing generic products to you…and also enable us to provide you with special discounts for products relevant to your needs, and general health and wellness information that will beneÞt you specifically, should you indicate that you want to receive such information.


6. Why do I need separate bonus cards for each family member?
This allows us to provide specific information and offers at an individual level. Your personal health information is protected and therefore we need your permission to furnish health information tailored to your specific needs. Children under the age of 18 can be attached to their parents’ bonus cards. Individuals over 18 must have their own cards, or the cardholder’s permission to use his or her card for other household members.


7. Is the Live Better!Wellness Club upgrade tied into Bonus Club databases, or are purchases scanned on each card held as separate records?
The Live Better!Wellness Club upgrade information is integrated with your bonus card information. Therefore once you have the upgrade your club card is good at all A&P, Waldbaum’s, SuperFresh and Super Foodmart stores.


8. Do I have the option to not provide personal medical information and still receive discounts on the card?
Yes, that aspect of the program is optional, so it is your choice whether to participate and provide that information. Also note that at your request, we can remove you from the program at any time. Your personal health information is protected by law; therefore we need your express permission to provide you with health and wellness information that addresses your individual health needs.


9. Can the Live Better! Wellness Club coupon that makes up for the $4.99 introductory fee be used in the pharmacy, or only at the regular checkout?
The $4.99 electronic coupon cannot be used for prescriptions, alcohol or tobacco. Therefore it is programmed only for use at the regular checkouts at the front end.


10. Do health profiles have to be filled out for children?
Assuming you participate in this aspect of the program (see question #4), information regarding children under the age of 18 is provided by parents or guardians at their discretion. For children 18 years of age or older, participation and provision of information is determined at their own discretion.


11. What products sold outside the pharmacy will be discounted through the Live Better! Wellness Club?
The range of products available through the program at any given time is extensive, and will be listed on our web pages at all times. In the stores, items discounted through the program will be designated with clearly marked “Live Better! Wellness Club” tags, so you can easily identify them. New products and other offers will be added on an ongoing basis.


12. How does this program relate to/interact with my regular prescription benefits, or any other discounts such as AARP?
This program will not replace, or add to, your regular prescription beneÞts. It is a program for customers without prescription coverage or other discounts.


13. How frequently will card program sale items change?
Items will remain on sale for a minimum of 8 weeks, and in many cases run continually until February 8, 2008.


14. I don’t own a computer. How would I receive coupons, recipes and other information made available by the program?
The discounts and offers are directly tied to your bonus card so you do not need paper coupons. Recipes and other information can be mailed to your home if you indicate that requirement on your application.


15. How can product and other offers be targeted to me based on personal health records, when privacy laws regarding such information are so strict?
These offers can only be provided to you if you expressly authorize us to do so on your application. Again, this portion of the program is optional, and also reversible at your direction should you opt in, but later decide you no longer want to provide and/or receive this information.


16. If I don’t provide personal medical information, what benefits would I receive with this card? Would I still get product discounts? Would they be e-mailed to me?
If you don’t provide personal medical information you will still receive all the product discounts. You just will not receive any information specific to any health or medical conditions that you did not provide to us or have not had a prescription for.


17. If this is an upgrade to my club card, what happens if I don’t have a club card?
If you do not have a club card you must first apply for one at the customer service desk. You would then bring your new card back to the Pharmacy department and join the Live Better!Wellness Club for $4.99. You will immediately receive the activation coupon for $4.99 o" your next $25 grocery order. There will be a five–day delay before you will receive all the other product discounts.


18. What if my spouse and I want to share our health information on a single card, but we each want our own separate prescription benefit? Do we both have to pay for cards?
Yes. To receive personal prescription benefits, each individual patient will need his or her own insurance card. Therefore you would each have to apply for a separate card and pay the $4.99 upgrade fee to get individual prescription coverage.
